Special Projects Director

DESCRIPTION

The Director of Special Projects for Barbour & Floyd is responsible for organizing objective and developing business strategies with the Executive Director and executive management team that are in-line with the agency strategic plan and coordinating staff efforts to implement said strategies.

ESSENTIAL DUTIES AND RESPONSIBILITIES ( OTHER DUTIES MAY BE ASSIGNED )
    • Launching the Integrated Primary Care project at Barbour and Floyd Medical Associates
    • Focused on growth, sustain ability and documentation of health objectives as outlined in DMH proposal
    • Working collaboratively with FQHC or other health care systems to organize the scheduling of clients
    • Improve the health of the population served and develop a system to capture data that will advance health care objectives
    • Improve the experience of each individual
    • Conversant with the SAMHSA PBHCI proposal and adapting those metrics that are relevant
    • Establish dialogue with other organizations that have performed an integrated primary care model
    • Establish well-being and wellness groups that are consistent with an integrated primary care and behavioral integration model
    • Coordinating client participation with other systems that promote wellness and well-being
    • Establish a training schedule for staff that encompasses principles of health navigation and care coordination
    • Encourage staff to participate in the healthcare promotion of clients
    • Providing Clinical Services at Barbour and Floyd Medical Associates
    • Establishing Clinitrak (EHR) proficiency
    • Knowledge of opening a client chart and proficiency in documenting treatment plans, assessments and progress notes
    • Keeping all clinical services documentation
    • Knowledge of housing strategies, inclusive of voucher use and applications
    • Empowerment Congress participation
    • Assist the CEO with the 2nd district Empowerment Congress
    • Prepare minutes of the Empowerment Congress
    • Assist the CEO with Mental Health Committee Empowerment Congress events and planning
    • Engage with Empowerment Congress Members
    • Organizational Development
    • Work with the CEO to implement organizational assessment needs
    • Work with CEO to establish a community defined evidence model for LGBTQ individuals and families in SPA6
EDUCATIONAND / OR EXPERIENCE

Master's degree (M.A.) or equivalent; or four to ten years related experience and or training; or equivalent combination of education and experience.

SPECIAL SKILLS AND KNOWLEDGE

Required computer skill: Proficient in Microsoft Word Preferred license or certification: MSW.

REQUIREMENTS

Must have own reliable source of transportation readily available..

Valid California Driver's License / Pass Criminal Livescan
Background check and MVR report.

These include Recovery, Resiliency, Reintegration (RRR), Full Service Partnership (FSP), and the Prevention & Early Intervention services for Adults & Older Adults(PEI)..
